Week 12 postpartum: twelve weeks, and what comes after
General guidance, not medical advice. Follow your own provider’s clearance. Read our Medical Disclaimer and Editorial Policy.
Twelve weeks is the conventional end of the fourth trimester and, in ACOG's framing, the point by which a comprehensive postpartum review should have happened. It is a boundary in the calendar rather than in the body: pelvic floor and core strength commonly keep improving for months past it.
If pain, leaking, heaviness, midline separation or low mood are still present, this is the appointment to raise them at. None of them are things to simply accept because twelve weeks have passed.
What is most worth attending to
A proper review rather than a graduation.
Movement that is generally appropriate at week 12
This is what MatraBloom would draw on to build a session at this point in recovery. It differs by birth type because a caesarean is abdominal surgery, so anything loading the abdominal wall is held back longer.
After a vaginal birth
- Breathing — Diaphragmatic Breathing, Cool-Down Breathing, Restorative Breathing
- Gentle mobility — Neck & Shoulder Rolls, Ankle Circles, Heel Slides, Seated Spinal Twist, Standing Calf + Hamstring Stretch, Cat-Cow (Supported), Child’s Pose, Standing Side Stretch
- Pelvic floor — Gentle Pelvic Tilts, Seated Pelvic Floor Squeeze, Wall Hip Hinge
- Strength — Wall Slides, Glute Bridges, Standing Leg Lifts, Seated Rows (Towel), Wall Squats, Side-Lying Leg Lifts, Wall Push-Offs, Band Pull-Apart, Seated Band Row, Banded Glute Bridge, Supported Dumbbell Row, Seated Shoulder Press, Goblet Squat

If you had a caesarean
Direct core work generally becomes appropriate after a caesarean around now — a full four weeks later than after a vaginal birth, because the abdominal wall was surgically opened rather than only stretched.
